Health, Consent & Release - RockRidge
Every person that you bring to camp must fill out this form completely (including all staff and volunteers). Check these forms as students turn them in to you and make sure they are properly filled out and signed before you head to camp. If students are missing anything, they will have to sit out from camp activities until the correct information is faxed into camp. Please bring 3 copies of your Health, Consent & Release Forms with you! (1 for YFCamp, 1 for RockRidge, and 1 to keep for your own records.) Housing Form - RockRidge
Fill out this form to make your housing requests and let us know about any kind of special needs that you or your campers may have. This includes housing needs, dietary needs, etc. Download the RockRidge Housing Form **Please e-mail this form to camp@yfc.org by NOON Mountain Time on Monday, June 7. A-Form - RockRidge
This form tells us the name of every person you are bringing to camp. This is your final list. E-mail this form to camp@yfc.org by NOON Mountain Time on Friday, June 18th. If you make any changes after this, bring the final list with you to camp.
